- Difficultly in performing activities of daily living, ADLs
- Unrelieved pain
- Shortness of breath or fatigue at minimal exertion • Multiple emergency department visits or hospitalizations
- 10% weight loss in a 6 month period
- Recurrent falls with injury
- Decreased tolerance and physical activity
