• What is Palliative Care?

    Palliative care is specialized medical care for people with serious or terminal illnesses. The support focuses on holistic care and providing relief from the symptoms, pain, and stress caused by serious illness. The goal is to improve the quality of life for both the patient and their family while reducing the emotional effects of illnesses. Home-based palliative care has also proven to provide significant cost savings through reduced hospital admissions and reduced usage of emergency services.

  • What does a home-based palliative care team do?

    The team consists of board-certified physicians, nurse practitioners, ambulatory nurses, registered dieticians, clinical therapists, spiritual guides/chaplains, and patient concierges all with disease-specific expertise. Our palliative care programs are tailored to the specific needs of each patient and include pain management, symptom control, crisis avoidance, and advance care planning. Our patient concierge facilities authorizations of medications, DME, and other services such as transportation to and from appointments. We liaise with PCPs and specialists to report on activity and ensure plans of care are followed. Our patients have access to our 24-7 nurse line, and we offer all of our services via telehealth.

  • Can I continue curative treatment in conjunction with palliative care?

    Yes! Unlike hospice, palliative care is supposed to work in conjunction with curative treatments. A large part of our program is tailored support.
